Auto And Third Party Insurance
We will submit the claim for you if you can provide a claim number and appropriate insurance information. However, if you do not bring the necessary information with you the day of your appointment, payment for service is your responsibility. At your visit a signed Personal Injury Protection (PIP) form will be required. We do not bill attorneys for services provided.


Insurance Form Submission
As a courtesy to our patients, we will submit your insurance forms at no additional charge to you. It is your responsibility to bring all of your insurance data to your appointment. However, in the event of non-payment from your insurance company, payment is your responsibility.


Disability Insurance
There will be a fee for completion of disability paperwork. Forms cannot be completed while you wait. Please allow 7 to 10 working days for completion. Payment for this service is due upon completion of your paperwork.


Prescriptions
For prescription refills, please contact your pharmacist during regular office hours. The pharmacist will contact our office for authorization of your refills. As practice policy, we do not refill narcotic drug prescriptions after regular office hours.


What is HIPAA?
HIPAA is the Health Insurance Portability and Accountability Act of 1996 enacted as Public Law 104-191. In addition to addressing insurability of pre-existing conditions and the continuation of health insurance coverage for individuals changing employers or employment status, congress also utilized this law to reform the health care system and provide uniform standards for handling medical records. Because every health care provider practice is unique, all guidelines from the government only define what needs to be accomplished. The administration falls back to the medical industry and private sector to decide how procedurally to address the HIPAA guidelines. Specific steps to compliance vary widely based on the diverse state of systems and procedures providers use to collect, store, maintain, transmit, and/or report medical records and billing information.
